Thank you for participating, our considering to participate in our online survey. The feedback we receive is valuable information used to help guide policy makers. As a thank you for your time, we are hoping to provide you with either a $10 electronic amazon gift card or a chance to win one of forty $50 electronic amazon gift cards.
If you decide you prefer the $10 dollar gift card you will receive your electronic amazon gift card within two weeks of completing the online survey. You will receive the gift card via your email we contacted you from. There are a limited amount of $10 gift cards. The first 1,000 respondents that select they want a $10 gift card will receive the gift card. After 1000 gift cards have been given out respondents will only have the option to enter into the online raffle. Once this occurs, participants will be notified that all $10 are no longer available.
If you decide to enter into the online raffle you will be notified when the online drawing will occur via the email we contacted you at. The raffle will be conducted using a free online raffle services. Only winners will be notified that they have won, and will be sent a $50 gift card via the email we contacted you at from amazon. The online raffle will happen after a sufficient number of survey responses have been collected. The researchers predict data collection will end by April and the raffle drawing will occur the first week of May 2024. Please check back here to see if the online raffle has occurred or if there is a change in the date of the expected drawing of the online raffle.
